Trilogy Health Services is committed to serving our communities through providing a full continuum of senior health and senior living services.

Trilogy Health Services communities offer a full range of personalized senior living services, from independent and assisted living, to skilled nursing and rehabilitative services.

TriStar Services is the Ancillary Services Division of Trilogy Health Services. TriStar is comprised of an institutional long term care pharmacy, a rehabilitation therapy service company and community based home care services. TriStar companies include PCA Pharmacy, Paragon Rehabilitation, Inc., and Vibrant Home Health.

The Trilogy Foundation is a nonprofit corporation established in Kentucky in 2007. It is tax-exempt as a private foundation under Section 501 (c)(3) of the Internal Revenue Code of the United States. The Trilogy Foundation provides assistance in a number of ways including:

• Awarding Educational Scholarships to deserving employees,
• Providing assistance to employees during times of crisis or natural disasters,
• Providing Community Education through Flu Immunization Clinics and Blood Pressure Screenings; and,
• Providing support through Charitable Giving to local organizations
